If you're living with migraine, you know it can be more than just head pain. It can be an emotional rollercoaster—especially when fear takes the wheel.

After almost 20 years of living with migraine, I’ve found that fear—specifically fear of migraine—can be just as limiting as the physical symptoms themselves. But here’s the good news: You can break through that fear. And when you do, everything starts to shift.

What is the Fear of Migraine?

Fear of migraine—also known as cephalalgiaphobia—is a very real, documented phenomenon. It’s the persistent fear of the next migraine attack, even during pain-free periods. This fear can silently control your life:

  • “What if I can't work today?”

  • “Will I have to cancel plans again?”

  • “Can I ever live my purpose if migraine keeps holding me back?”

This fear doesn’t just affect your emotional well-being. Research shows it can also contribute to the chronification of migraine, where episodic migraine becomes chronic.

How Fear Contributes to Chronic Migraine

Studies published in The Journal of Headache and Pain (2007, 2013) reveal a strong correlation between fear of migraine and increased migraine frequency. Why?

  • Fear may lead to medication overuse, taking acute meds more often than needed out of panic.

  • The brain’s fight-or-flight system becomes overactive, heightening sensitivity to pain.

  • Constant anxiety wires your brain to expect pain—so it keeps delivering it.

Simply put, the more fear your brain associates with migraine, the more it keeps the pain loop going.

How Fear Shrinks Your Life

Fear of migraine can creep in slowly and quietly. Maybe you stop going out with friends. Or skip exercise because you’re worried about triggering an attack. Before you know it, your world becomes smaller. Less joy. More isolation.

But here's a truth worth repeating: fear is not your fault. It's your brain trying to protect you. The problem is, it's using outdated tools—your primitive amygdala—to make decisions. What we want is to activate your prefrontal cortex—your more evolved, discerning brain.

That’s where the healing starts.

Breaking through fear doesn’t mean jumping headfirst into a triggering environment. It means taking small, safe steps back into your life:

  • Take a short walk around the block.

  • Drive for five minutes and come home.

  • Meet a friend for coffee in a calm setting.

Each small win tells your brain, “I’m safe.” Over time, these add up to powerful transformation.

From Surviving to Thriving: A Client Story

One of my coaching clients recently attended a concert for the first time in years. She brought earplugs, had an exit plan, and even took a small dose of medication beforehand. But the best part? She enjoyed herself—and didn’t end up with a migraine attack.
